Service Coordination. Helping Manage Your Available Benefits

Service Coordination Unlimited (SCU) enables adults with physical disabilities to live as independently as possible in their own homes.  Service coordinators assist individuals in coordinating their services based on their individual needs and available supports.

Since services are based on the individual’s needs, service coordinators can also help the person apply for other services in the community that they may be interested in.

SCU provides Service Coordination to anyone currently enrolled in the Office of Long Term Living Waivers including ACT 150, OBRA, Independence, COMMCARE, and Attendant Care in all of the regions they serve. The Pittsburgh Offices also serve individuals on the Bureau of Autism’s Autism Waiver.  For those who may not be eligible for these waivers, SCU provides services to individuals supported through trusts, settlements, private pay, or third party entities.

Service coordinators can offer the following services:

  • Service Coordination
  • Environmental Modifications
  • Miscellaneous Durable Medical Equipment
  • Transportation, PERS (life call system)
  • Community Transition Services

In addition to theses services, service coordinators can also connect individuals to other resources in the community such as:

  • Personal Care, models include agency, consumer, or Services My Way
  • Respite
  • Educational Services
  • Community Integration
  • Peer Counseling
  • Night Supervision
  • Adult Day
  • Supported Employment
  • Prevocational Services
