SCU has impacted my independence in many ways. Through transportation services, I am now able to go to many activities such as church, choir practice, the library, museums, the mall etc. I am able to access the community and increase my interactions with others which has opened me up to feel more like an accepted member of society.
Community HealthChoices — Managed Care Program for Pennsylvania’s Aging and Disabled
Community Health Choices (CHC) is a mandatory managed care plan in Pennsylvania that was developed with the intent to improve coordination of physical and behavioral health systems, as well as Long Terms Services and Supports (LTSS).
Community Health Choices aims to improve the quality of in-home supportive services for people who require long term care and wish to avoid becoming institutionalized. The program offers a wide array of services to support adults with physical disabilities, as well as the aging population in PA, with their choice to continue to live independent lives. CHC is also designed to assist nursing home residents with transitioning back to the community.
All eligible individuals have to choose a Managed Care Organization (MCO) to coordinate their CHC care plan.
